We are seeking a highly skilled and compassionate Mental Health Social Worker I to join our team at Fraser Health Authority. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients suffering from acute episodes of mental illness and substance misuse/dependence.
Key Responsibilities- Conduct Comprehensive Assessments: Conduct thorough social work and psycho-social assessments of patients and their families, using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Develop and Coordinate Care Plans: Collaborate with the interdisciplinary team to develop and coordinate care plans, ensuring optimal continuity of care and effective utilization of patient resources.
- Provide Education and Support: Educate and provide consultation and support to patients, families, and healthcare professionals on issues related to mental health, substance misuse, and addiction.
- Collaborate with Community Agencies: Communicate and collaborate with mental health and related community service agencies, family physicians, and other healthcare professionals to ensure seamless care coordination.
- Maintain Accurate Records: Maintain accurate and up-to-date patient records, documenting assessments, charting patient information, and preparing progress notes and reports as required.
- Education and Experience: Master's Degree in Social Work from an accredited school of Social Work, plus one year's recent experience in a mental health treatment setting.
- Registration and Certification: Current full registration with the BC College of Social Workers.
- Skills and Abilities: Comprehensive knowledge and understanding of emotional issues related to mental illness and addiction, ability to make effective referrals, and strong collaborative skills in working with community health and social agency personnel.
