Social Worker I MSW, In-Patient Psychiatry, Mental Health Specialist

We are seeking a highly skilled and compassionate Clinical Social Worker to join our In-Patient Psychiatry team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive social work services to patients and their families, promoting optimal care and support throughout their hospital stay and beyond.
Key Responsibilities- Conduct comprehensive social work assessments of patients and their families, identifying needs and developing individualized care plans.
- Provide counseling and therapy services to patients and their families, addressing emotional and psychosocial issues related to mental illness and addiction.
- Collaborate with healthcare professionals and community agencies to ensure seamless care transitions and optimal community support.
- Develop and deliver educational programs and support groups for patients and their families, promoting mental health awareness and recovery.
- Participate in interdisciplinary team meetings and contribute to the development of care plans and community services.
- Maintain accurate and comprehensive patient records, documenting assessments, progress notes, and treatment plans.
- Stay up-to-date with best practices and research in mental health social work, applying knowledge to inform clinical practice and program development.
- Master's degree in Social Work from an accredited program.
- Current registration with the British Columbia College of Social Workers.
- Minimum one year of experience in a mental health treatment setting.
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and healthcare teams.
- Strong knowledge of community resources and services, with ability to make effective referrals and coordinate care.
- Ability to work independently and as part of an interdisciplinary team, with strong problem-solving and critical thinking skills.
