Job Title: Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in providing clinical social work services to clients and their families in the Timber Creek mental health program.

Key Responsibilities:
  • Provide comprehensive therapeutic clinical counselling services to clients and their families, community supports, and interdisciplinary treatment teams.
  • Maintain an active role in planning for care during the hospital stay, and facilitating optimal discharge planning.
  • Conduct comprehensive social work and psychosocial assessments of clients, and families by interviewing the client and/or family, obtaining relevant information, gathering social data regarding the client and/or family and interpreting the data using a variety of modalities.
  • Educate and provide consultation and support to clients, families, and healthcare professionals on issues related to high risk factors, recovery and current community practice in the field of mental health, child protection, adaptation to illness and disability, protection of vulnerable adults, chemical dependency, community living and support services.
  • Maintain client records by documenting client assessments, charting client information, preparing progress notes and reports in accordance with established standards, policies and procedures to meet regulatory requirements and documenting for future reference.
Qualifications:
  • Master's Degree in Social Work (MSW) from an accredited school of Social Work.
  • Two (2) years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience.
  • Current full registration with the British Columbia College of Social Workers.
  • Valid Class 5 BC driver's license with access to personal vehicle for business purposes.
Skills and Abilities:
  • Comprehensive knowledge and understanding of emotional issues related to mental illness and addictions and client and family cultural dynamics.
  • Comprehensive clinical counselling skills including crisis counselling, brief therapeutic interventions and group work applicable to clients and families.
  • Knowledge of legislation applicable to social work in mental health programs, and ability to apply the principles within relevant Mental Health Acts including the Adult Guardianship Act.
  • Demonstrated ability to organize and prioritize workload in a complex environment.
  • Ability to educate and provide consultation to clients, families and healthcare professionals.
  • Ability to provide leadership to the interdisciplinary team in care planning for the effective utilization of client resources.
  • Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with community health and social agency personnel.
  • Ability to work effectively with others, including working effectively in a multicultural environment, independently, and as part of an integrated interdisciplinary team.
  • Demonstrated ability to communicate effectively both verbally and in writing with clients, families and staff and community agency personnel.
  • Ability to develop, coordinate and deliver in-services, presentations, psychoeducational and support groups.
  • Ability to initiate, develop and carry out research activities.
