About the Role

We are seeking a skilled Registered Nurse to join our team as a Community Health Practitioner. In this role, you will provide high-quality patient care in various settings, working with patients of all ages and complexity of care needs.

Key Responsibilities

  • Conduct thorough assessments of patients' individualized needs using appropriate tools and techniques.
  • Develop and implement personalized care plans, continuously monitoring and updating patient records.
  • Build strong relationships with patients and their families, using a collaborative approach to engage and empower them.
  • Maintain accurate and up-to-date patient records, reflecting patient encounters and care plans.

Requirements

  • Registration with the CNO as a Registered Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability, flexibility, and a passion for making a difference in your community.
  • Ability to travel to and from patient residences within a designated area.
  • Willingness to participate in on-call coverage, including after-hours, to ensure patient safety and meet care needs.
