About the Role

The Canadian Mental Health Association Toronto Branch is seeking a skilled Transitional Support Worker to join our team. As a key member of our Transitional Rehabilitation Housing Program 2 (TRHP 2), you will play a vital role in supporting individuals with complex mental health and addictions issues in their transition to permanent housing.

This is a unique opportunity to work with a multidisciplinary team, providing on-site direct service provision, group facilitation, and social recreational programming. You will work closely with clients to build life skills, assess mental status, and provide supportive counseling. Your strong organizational skills and experience in group facilitation will be essential in leading residents' meetings, conflict resolution, and crisis prevention/intervention.

Responsibilities
  • Provide on-site direct service provision, including Activities of Daily Living (ADLs) support and life skills training
  • Facilitate group programs and social recreational activities
  • Lead residents' meetings and provide conflict resolution and crisis prevention/intervention
  • Assess mental status and provide supportive counseling
  • Maintain accurate documentation and coordinate site maintenance duties
Requirements
  • Minimum college diploma in Social Work/Social Sciences, Recreation Therapy, or Life Skills
  • Minimum three years of experience working with adults with complex mental health and addictions issues
  • Strong organizational skills and experience in group facilitation
  • Excellent written and oral communication skills
  • Ability to work within a multidisciplinary team and independently
