Dietitian, Ambulatory Care Specialist

We are seeking a highly skilled Registered Dietitian to join our team at North York General Hospital. As a member of our inter-professional healthcare team, you will provide safe, effective, and ethical care for patients and their families.
Key Responsibilities- Provide client-centered assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families.
- Develop and implement nutritional care plans that meet patient needs in collaboration with the inter-professional team.
- Take on leadership in developing and participating in center-specific and corporate patient care initiatives related to diabetes.
- Advocate for patients and families to ensure their rights are recognized and appropriate services are provided.
- Formulate and facilitate discharge plans with patients, families, and healthcare teams, utilizing community resources.
- Successful completion of a 4-year degree in human nutrition and dietetics from a university program accredited by the Partnership for Dietetic Education and Practice (PDEP) or recognized equivalent.
- Successful completion of an accredited dietetic internship or equivalent supervised practical training.
- Member in good standing with the College of Dietitians of Ontario.
- Certified Diabetes Educator.
- 1-year recent clinical experience and expertise in diabetes management and education, with a focus on specialized and complex diabetes care.
