Job Overview
The Nutrition Specialist in Ambulatory Care plays a vital role within the inter-professional healthcare team at North York General Hospital. This position is dedicated to delivering safe, effective, and ethical nutritional care to patients and their families.Position Responsibilities
As a Registered Dietitian, you will be responsible for:
Conducting comprehensive assessments of the emotional, physical, cognitive, and social needs of patients and their families. Providing tailored care to patients with diabetes and their caregivers, ensuring a patient-centered approach. Leading and participating in diabetes-related patient care initiatives specific to the center. Developing nutritional care plans in collaboration with the inter-professional team to meet individual patient needs. Documenting assessments and progress notes in patient health records, monitoring and evaluating outcomes to communicate effectively with the healthcare team. Interpreting therapeutic diet prescriptions according to best practice guidelines. Advocating for patients and families to ensure their rights are upheld and appropriate services are accessible. Facilitating discharge planning with patients, families, and the healthcare team, utilizing community resources effectively. Participating in medical team meetings and inter-professional rounds, serving as a resource for staff. Counseling individuals and groups on both normal and therapeutic nutrition. Supporting the education of dietetic students through clinical rotations and evaluations based on established competencies. Maintaining a thorough understanding of hospital and community resources. Promoting evidence-based care through the implementation of initiatives aligned with the organization's strategic goals. Engaging in continuing education for personal and team development. Actively participating in Clinical Nutrition Professional Advisory Committee meetings and representing Clinical Nutrition on relevant committees.
Qualifications
To be considered for this role, candidates should possess: A 4-year degree in human nutrition and dietetics from an accredited university program. Completion of an accredited dietetic internship or equivalent supervised training. Membership in good standing with the College of Dietitians of Ontario. Certification as a Diabetes Educator. A minimum of one year of recent clinical experience in diabetes management and education, particularly in specialized diabetes care. Preferred certification as an Insulin Pump Trainer. Membership with Dietitians of Canada is an asset. Current teaching and counseling experience. Proficiency in electronic documentation and computer skills. Bilingualism in French and English is an asset.
