Dietitian, Ambulatory Care Specialist

The Registered Dietitian is a key member of the North York General Hospital inter-professional healthcare team. This role involves providing safe, effective, and ethical care for patients and their families. The successful candidate will be responsible for nutritional care for patients in accordance with established standards of dietetic practice.
Key Responsibilities- Provide client-centered assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families.
- Develop and implement self-management directed care plans for patients with diabetes and their families/caregivers.
- Take a leadership role in developing and participating in centre-specific and corporate patient care initiatives related to diabetes.
- Prepare dietetic nutritional plans of care that meet patient needs in collaboration with the inter-professional team.
- Chart written assessments and progress notes on patient health records, monitor, reassess, and evaluate for optimal outcomes.
- Recommend and interpret therapeutic diet prescriptions/nutrition support orders using best practice guidelines.
- Advocate for patients/families and ensure their rights are recognized and appropriate services are created/provided in collaboration with the inter-professional team.
- Formulate and facilitate discharge plans with patients, families, and healthcare teams, utilizing community resources.
- Participate in medical team meetings and inter-professional rounds, acting as a liaison and resource person for all staff.
- Counsel individuals and groups in normal and therapeutic nutrition programs.
- Support the education program for dietetic interns by directing, supervising, and evaluating their performance.
- Maintain excellent knowledge of and close liaison with hospital and community resources.
- Promote best practice and evidence-based care through the implementation of initiatives aligned with the organization's strategic direction.
- Support the development of program/unit goals, objectives, policies, procedures, and quality assurance programs.
- Participate in continuing education activities for self, peers, and members of the inter-professional healthcare team.
- Attend and actively participate in Clinical Nutrition Professional Advisory Committee meetings and provide Clinical Nutrition representation on corporate/unit committees, as required.
- Successful completion of a Foods & Nutrition program accredited by the Partnership for Dietetic Education and Practice (PDEP) or the Accreditation Council for Education in Nutrition and Dietetics (ACEND).
- Successful completion of an internship or practicum program or a graduate program that is accredited by the Partnership for Dietetic Education and Practice or completion of an internship or practicum that is accredited by the Accreditation Council for Education in Nutrition and Dietetics.
- Registered member, in good standing, of the College of Dietitians of Ontario.
- Certified Diabetes Educator preferred.
- Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education with a focus in specialized and complex diabetes care.
- Certified Insulin Pump Trainer preferred.
- Experience in Paediatric Diabetes Management preferred.
- Membership with Dietitians of Canada preferred.
- 2 years' experience in complex nutrition support preferred.
- Current teaching experience and counseling background.
- Current experience with electronic documentation along with computer proficiency.
- Current knowledge of normal and therapeutic nutrition, enteral feeds, and TPN.
- Bilingualism in French/English an asset.
