Job Title: Dietitian, Ambulatory Care

North York General Hospital is seeking a highly skilled and experienced Dietitian to join our Ambulatory Care team. As a key member of our inter-professional healthcare team, you will provide safe, effective, and ethical care for patients and their families.

Key Responsibilities:
  • Provide client-centred assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families.
  • Develop and implement personalized nutrition care plans that meet patient needs in collaboration with the inter-professional team.
  • Lead and participate in centre-specific and corporate patient care initiatives related to diabetes.
  • Chart written assessments and progress notes on patient health records, monitor, reassess, and evaluate for optimal outcomes.
  • Recommend and interpret therapeutic diet prescriptions/nutrition support orders by utilizing best practice guidelines.
  • Advocate for patients and families to ensure their rights are recognized and appropriate services are created/provided.
  • Formulate and facilitate discharge plans with patients, families, and healthcare teams, utilizing community resources.
Requirements:
  • Successful completion of a Foods & Nutrition program accredited by the Partnership for Dietetic Education and Practice or the Accreditation Council for Education in Nutrition and Dietetics.
  • Registered member, in good standing, of the College of Dietitians of Ontario.
  • Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education.
  • Certified Diabetes Educator preferred.
