Care Navigator Specialist

We are seeking an experienced Care Coordinator to join our team at Home and Community Care Support Services. As a Care Navigator Specialist, you will play a critical role in assessing patient care needs, determining eligibility for services, and developing individual care and service plans.
Job DescriptionAs a key member of our multidisciplinary team, you will be responsible for linking patients with service providers, coordinating and monitoring care plan delivery, establishing helping relationships with patients and their families, and balancing patient needs and choices with available resources.
You will work collaboratively with patients, caregivers, primary care providers, hospitals, long-term care and retirement homes, and service providers to deliver responsive, accessible, integrated, patient-centred care.
Required Skills and Qualifications- A membership in good standing with the applicable regulatory body (College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers, or College of Audiologists and Speech Language Pathologists of Ontario)
- Three to five years of recent experience in children's health with a focus on children and youth with medical complexity and high risk/high utilization conditions
- An understanding of Ontario's provincial supports and strategies available to children with medical complexity and other conditions (Complex Kids Ontario, Ministry of Health, Ministry of Children, Community and Social Services, and Ministry of Education)
- Knowledge of community services and resources for children and youth within Mississauga Halton Geography
- Clinical expertise providing patient and family-centred care, including critical thinking, ethical decision-making, and collaboration with various stakeholders
- Leadership abilities to support inter-professional teams and caregivers
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
- Able to accurately complete required documentation, reports, and forms
- A valid driver's license and access to a reliable vehicle
- Proficient in a Windows environment

$60,000 - $80,000 per year, depending on experience and qualifications.
