About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality nursing care to patients in a clinic setting. As a Clinical Care Specialist, you will be responsible for assessing, planning, and implementing individualized care plans that meet the unique needs of each patient.

Duties and Responsibilities

  • Provide patient-focused, safe, holistic nursing care in a clinical setting
  • Ensure that nursing care provided to the patient is based on current and best practices to achieve successful outcomes
  • Continually assess, anticipate, identify and respond to new and changing needs of patients

Qualifications and Requirements

  • Minimum 2 years of community nursing experience
  • Currently registered and in good standing with the College of Nurses of Ontario (CNO)
  • Ability to work independently in managing a caseload
  • Proficiency in English, both verbal and written required (Additional language skills considered an asset)
  • Experience with infusion therapy, mental health, palliative care, and wound care is an asset
  • Excellent patient assessment and teaching skills
  • Demonstrated problem-solving and critical thinking skills
  • Physical ability to perform the day-to-day duties described in the job description
  • Basic proficiency in Word, Excel and ability to learn proprietary software

Benefits and Compensation

We offer a competitive salary range of $85,000 - $110,000 per annum, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, and a generous pension plan.
