Job Summary


We are seeking an experienced Registered Nurse (RN) to join our team as Home Care Specialist, Palliative Nurse. This role will lead an integrated palliative care team within a designated neighbourhood, managing day-to-day coordination of services to ensure continuity of care and seamless processes.



About the Role


In this role, you will:



  • Lead an integrated palliative care team to assess, plan, and coordinate patient care plans that support patients to die in their preferred place of death.
  • Collaborate with your team to provide high-quality care, including pain and symptom management, advanced clinical skills, end-of-life care, and interdisciplinary collaboration.
  • Oversee indirect care coordination activities provided by regulated and unregulated health professionals on your care team.
  • Support your team to provide emotional support and counselling, ethical and legal understanding, assessment and planning, education, and advocacy.
  • Build trusting relationships with patients and caregivers to understand their goals and needs.
  • Effectively manage internal and external stakeholder relationships.
  • Address quality, safety, and risk issues, developing improvement plans as needed.


Requirements


To be successful in this role, you will require:



  • A valid RN registration with the College of Nurses of Ontario.
  • A minimum of 5 years of nursing experience in a community setting and 2+ years in a management role.
  • Experience working with palliative care patients and program development, implementation, and evaluation.
  • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or equivalent experience.
  • A valid G driver's license and reliable vehicle.


About the Salary


The estimated annual salary for this role is $115,000-$135,000 based on location and industry standards.
