About the Role: We are seeking a highly skilled Patient Care Coordinator to join our team at Home and Community Care Support Services. As a Patient Care Coordinator, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to individuals in our community.
Key Responsibilities:
- Collaborate with patients, families, and healthcare providers to develop and implement comprehensive care plans that meet the unique needs of each individual.
- Conduct thorough assessments to identify patient needs and develop strategies to address them.
- Coordinate and facilitate care delivery, including arranging for services and supporting patients and families in navigating the healthcare system.
- Maintain accurate and up-to-date records of patient information and care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care delivery.
Requirements:
- Membership in good standing with a relevant regulatory body.
- Minimum two years of recent experience in community health or a related field.
- Sound knowledge of the Ontario healthcare system and relevant legislation.
- Basic knowledge of and adherence to relevant legislation and regulations.
