About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Nurse Specialist in Wound, Ostomy and Continence Care. As a key member of our community nursing team, you will provide high-quality nursing care to patients in their homes, focusing on wound, ostomy and continence care management.

Key Responsibilities

  • Provide patient-focused, safe, and holistic nursing care to patients in their homes
  • Work collaboratively with other healthcare professionals to achieve successful patient outcomes
  • Continually assess, anticipate, and respond to new and changing patient needs
  • Travel between patients' homes within a specific geographical region

Requirements

  • Minimum of two years' experience as a Registered Nurse, with community nursing experience preferred
  • Currently registered and in good standing with the College of Nurses of Ontario (CNO)
  • Must have completed the 'Nurses Specialized in Wound, Ostomy and Continence Canada' (NSWOCC) course
  • Ability to work independently in managing your own caseload
  • Proficiency in English, both verbal and written required
  • Valid Ontario driver's license and reliable vehicle with willingness to travel
