Job Summary:

We are seeking a skilled Mental Health Specialist to join our Youth Intensive Case Management team at the Outreach Community Health & Services Centre in Kelowna, B.C.

Key Responsibilities:

  • Provide case management services to high-risk youth with problematic substance use, concurrent disorders, and/or mental illness.
  • Work collaboratively with an interdisciplinary team to develop and implement individualized care plans.
  • Conduct assessments, develop treatment plans, and provide counseling services to youth and their families.
  • Collaborate with community agencies and healthcare providers to ensure comprehensive care.
  • Participate in team meetings and contribute to the development of policies and procedures.

Requirements:

  • Previous experience working directly with youth and community Mental Health and Substance Use (MHSU) services.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and services.
  • Ability to maintain confidentiality and handle sensitive information.
