Job Summary

The Mount Carmel Clinic is seeking a skilled Community Outreach Specialist to join our team. As a key member of our coordinated service delivery team, you will work proactively with new and existing clients, offering a range of support and recovery services. Your primary focus will be on building relationships and advocating for clients, providing services in a non-judgmental and culturally appropriate manner.

Key Responsibilities

  • Develop and implement service plans that meet the unique needs of clients
  • Provide outreach services to hard-to-reach populations, including those with complex needs
  • Collaborate with interdisciplinary teams to ensure comprehensive support for clients
  • Advocate for clients' rights and interests, promoting their autonomy and self-determination
  • Conduct regular assessments and evaluations to ensure the effectiveness of services

Requirements

  • Minimum of 2 years' experience working in community services, preferably with vulnerable populations
  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
  • Strong understanding of the Housing First model and Assertive Community Treatment (ACT) methodology
  • Excellent verbal and written communication skills, with proficiency in the use of computer systems
  • Post-secondary education in a Social Services field (an asset)
