As a Transition Coordinator at Alberta Health Services, you will be part of a team of Transition Coordinators with an assigned portfolio/inpatient unit(s) including Emergency. Your role will involve assessing patients' needs in the context of the whole person, including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from various sources are essential for clinical decision-making. You will receive specialized training to provide you with the knowledge, tools, and resources to confidently care for your patients.

Description:

As a Registered Nurse (RN), you will provide a wide range of nursing services to patients, families, communities, and populations while ensuring their safety and well-being. You will utilize nursing processes through critical thinking, problem-solving, and decision-making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.

Key Responsibilities:

  • Completing comprehensive electronic needs assessments
  • Determining urgency/priority
  • Developing and implementing appropriate service plans
  • Coordinating service delivery across programs/settings
  • Liaising with Continuing Care staff and other community resources
  • Providing information and support to clients, families, and caregivers

Required Qualifications:

  • Completion of an accredited nursing education program
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)

Additional Required Qualifications:

  • A minimum of five years facility-based nursing experience, including geriatric experience
  • A minimum of two years' experience within community nursing (home care, supportive living, or facility living)
  • Effective time management and prioritization skills
  • Conflict resolution and strong communication skills

Preferred Qualifications:

  • Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech, and e-People
