Your Opportunity:

As a Transition Coordinator, you will be part of a team of healthcare professionals at the Royal Alexandra Hospital, working with patients and their families to assess their needs and develop comprehensive care plans. You will utilize your critical thinking and problem-solving skills to determine the best course of action for each patient, taking into account their physical and social environments. Your role will involve working with a multi-disciplinary team to coordinate care, communicate with other healthcare providers, and provide education and support to patients and their families.

Key Responsibilities:

  • Completing comprehensive electronic needs assessments
  • Determining urgency and priority
  • Developing and implementing service plans
  • Coordinating service delivery across programs and settings
  • Liaising with Continuing Care staff and other community resources
  • Providing information and support to clients, families, and caregivers

Requirements:

  • Completion of an accredited nursing education program
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • A minimum of five years facility-based nursing experience, including geriatric experience
  • A minimum of two years' experience in community nursing

Preferred Qualifications:

  • Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech, and e-People
  • Active Transition Coordinators with up-to-date certification may receive preference
