We are offering an engaging role as a Transition Coordinator within Alberta Health Services. This position is ideal for those who excel in navigating complex healthcare environments and making informed decisions. As a Transition Coordinator, you will be integral to the team, focusing on clinical evaluations for patients across various healthcare settings, including emergency care. Your responsibilities will encompass critical decision-making, thorough assessment of patient requirements, and the orchestration of care among diverse healthcare professionals.
Key Responsibilities:
- Performing detailed electronic evaluations of patient health
- Creating tailored service plans for individual needs
- Overseeing the implementation of service delivery
- Facilitating communication among healthcare providers and community services
- Offering guidance and support to patients and their families
Essential Qualifications:
- Completion of an accredited nursing program
- Active registration with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support certification
- A minimum of 5 years of nursing experience in a facility setting with a focus on geriatric care
- At least 2 years of experience in community nursing
Preferred Skills:
- Familiarity with software applications such as Microsoft Word and Meditech
- Current certification for Transition Coordinators
