We are seeking a skilled Transition Coordinator to join our team at Royal Alexandra Hospital. As a Transition Coordinator, you will play a vital role in clinical consultations for patients in various settings, including Emergency. This dynamic position involves complex decision-making, assessment of patient needs, and coordination of care across different healthcare providers.
Key Responsibilities:
- Conduct comprehensive electronic assessments to identify patient needs and develop effective service plans.
- Coordinate service delivery, ensuring seamless transitions and optimal patient outcomes.
- Communicate effectively with healthcare providers and community resources to ensure comprehensive care.
- Provide information and support to clients and families, promoting empowerment and independence.
Requirements:
- Graduation from an accredited nursing program.
- Active registration with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support certification.
- Minimum of 5 years of facility-based nursing experience with geriatric exposure.
- Minimum of 2 years of community nursing experience.
Preferred Qualifications:
- Proficiency in computer applications such as Microsoft Word and Meditech.
- Up-to-date certification for Transition Coordinators.
