CarePartners, a leading home healthcare provider in Ontario, is seeking skilled Community Support Workers (CSWs) to join our High Intensity Supports at Home (HISH) program. As a CSW with CarePartners, you will work closely with patients and their families to provide comprehensive support during transitional periods within the healthcare system.

Job Responsibilities
- Assist patients with daily living activities, including personal care, household tasks, respite care, medication assistance, and special functions
- Promote a safe environment by utilizing proper body mechanics, transfer techniques, and home safety practices
- Provide cognitive, social, and emotional support to patients
- Prepare nutritious meals and snacks, adhering to care plans and proper food preparation, storage, and hygiene practices
Requirements
- A strong desire to make a positive impact on patients' lives
- Exceptional interpersonal skills and ability to work independently and collaboratively as part of a team
- Availability to work non-traditional hours, including early mornings, evenings, and alternate weekends
- A valid driver's license and vehicle
- A valid certificate as a Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or enrollment in a Nursing program
- Current vulnerable sector check screening and a clear background check

Salary
$44,000 - $55,000 per year, depending on experience
