Job Description

CarePartners, one of Ontario's largest accredited home health care providers, is seeking a skilled Home Health Care Nurse to join our team in the Oshawa area. As a Visiting Nurse with our organization, you will have the opportunity to provide exceptional nursing services to patients in their homes, while enjoying flexible scheduling and a supportive work environment.

About the Role

This is a full-time position that involves developing and maintaining effective care plans for patients receiving community health services. You will work independently or in cooperation with the Ontario Health atHome to ensure comprehensive care for your patients. Your duties will include:

  • Providing one-on-one nursing services to patients in their homes
  • Communicating effectively with patients, family members, and interdisciplinary care teams
  • Performing clinical duties as required, including assessing patient needs and developing personalized care plans

  • A competitive salary range of $85,000 - $110,000 per year, depending on experience
  • PAID MILEAGE at $0.45 per/km PLUS travel time

  • Up-to-date mobile devices with current scheduling and virtual technology
Requirements

To be successful in this role, you must possess:

  • A Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills and excellent English verbal and written communication skills
  • A valid driver's license and vehicle, as regular travel is required
