Job OverviewWe are currently seeking a Registered Nurse Hospital to join our team of healthcare professionals in the Oshawa area. As a CarePartners Community Nurse, you will provide exceptional nursing services to patients in their homes, promoting independence and dignity.
Responsibilities:- Patient Assessment and Care Planning: Develop and maintain effective care plans for community health services, ensuring that patients receive comprehensive and personalized care.
- Nursing Services: Provide exceptional nursing services to patients in their homes, including assessments, interventions, and evaluations.
- Collaboration and Communication: Communicate and liaise with patients, family members, and other healthcare professionals as part of an interdisciplinary care team.
- Assess Patient Needs: Assess patients' needs and develop individualized care plans to ensure optimal outcomes.
- Provide Education and Support: Provide education and support to patients and their families to promote health and wellness.
- Collaborate with Healthcare Professionals: Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
- Document Patient Information: Document patient information accurately and thoroughly to ensure continuity of care.
- Current Registration: Current registration with the College of Nurses of Ontario (CNO)
- Driver's License: A valid driver's license and vehicle
- Excellent Communication Skills: Excellent English verbal and written communication skills
- Immunization Record: Current immunization record, including 2-step TB test

Estimated Salary:$85,000 - $110,000 per year
