Salary
We offer a competitive wage grid that recognizes nursing skillset, with an estimated salary range of $85,000 to $110,000 per year, depending on experience.
Job Description
As a Nurse Care Coordinator, you will work independently or in cooperation with Ontario Health atHome to develop and maintain effective Care Plans for Community Health Services. You will provide exceptional nursing services to patients both in-person and virtually, working in various settings including in-home, clinic, retirement home, or school.
Required Skills and Qualifications
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- A valid driver's license and vehicle as regular travel is required
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record including 2 step TB test
- To protect our patients, we require a current vulnerable sector check and a clear background check
