Job Summary
This Registered Practical Nurse position is an exciting opportunity to provide compassionate care to patients in their homes. As a member of our community nursing team, you will have scheduled visits to deliver high-quality patient care, working closely with RNs for more complex cases. Your autonomy and independence will allow you to build meaningful relationships with patients and their families.
About the Role
In this role, you will:
- Participate in scheduled home visits to assess and meet patient needs within your scope of practice;
- Provide safe and ethical nursing care using appropriate assessment tools and techniques;
- Maintain accurate and up-to-date patient records related to encounters and plans;
- Engage patients through collaborative approaches, building trust and rapport;

Requirements
To succeed in this role, you must possess:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing;
- A valid driver's license and access to a vehicle;
- Experience in delivering community care, an asset but not required;
- Reliability and flexibility to adapt to changing patient needs.
