Reconnect Community Health Services is a leading not-for-profit organization in Toronto, dedicated to providing high-quality support services for individuals with mental health and addictions concerns.

We are currently seeking an experienced Mental Health Worker to join our team. As a key member of our multidisciplinary team, you will provide on-site assessment, intake, discharge planning, and support to clients in West Toronto.

Job Description:
  • Conduct thorough client assessments and develop individualized support plans
  • Cordinate client access to short-term crisis beds and other community resources
  • Provide a range of individualized on-site supports, including housing assistance, financial aid, medical support, and advocacy
  • Liaise with other service providers, mobile crisis teams, hospitals, physicians, and community workers on behalf of clients

The ideal candidate will possess a University degree or Diploma in social services, with a minimum of two years' experience in direct service delivery to adults with serious mental illness and addictions. You will also have excellent communication skills, knowledge of the mental health system, and experience working effectively as a member of a team.

Salary and Benefits:

This full-time position comes with a competitive salary of $43,900 - $56,000 per year, plus comprehensive benefits, including paid sick leave, vacation time, and RRSP contributions.

Requirements:
  • University degree or Diploma in social services
  • Minimum two years' experience in direct service delivery to adults with serious mental illness and addictions
  • Excellent verbal and written communication skills
  • Knowledge of the mental health system and experience working effectively as a member of a team
