The Centre for Addiction and Mental Health is currently seeking a full-time, permanent Social Worker 2 to work in the Psychosis Coordinated Care Service (PCCS) located at 1051 Queen Street West. This position offers a competitive salary of $85,000 - $110,000 per annum, depending on experience.


About the Role


In this dynamic and rewarding role, you will be responsible for providing client-centered psychiatric care and relevant social work services to clients with complex mental health needs. As a member of an interprofessional team, you will work collaboratively to develop and implement individualized treatment plans that promote recovery, education, and stabilization of symptoms.


You will have the opportunity to conduct family assessments, offer family education, and provide counseling and support to families. Additionally, you will liaise with other services in the Complex Care and Recovery Program and the community to coordinate care and refer clients to appropriate services and resources.


Key Responsibilities



  • Conduct comprehensive assessments of clients' mental health needs and develop individualized treatment plans;
  • Provide client-centered psychiatric care and relevant social work services to clients with complex mental health needs;
  • Work collaboratively with the interprofessional team to develop and implement individualized treatment plans;
  • Conduct family assessments and offer family education and counseling;
  • Liaise with other services in the Complex Care and Recovery Program and the community to coordinate care and refer clients to appropriate services and resources.

Requirements


To be successful in this role, you must possess a Master's degree in Social Work and be a certified member of the Ontario College of Social Workers and Social Service Workers. You should also have three years' experience working in a mental health setting and demonstrate a strong knowledge of crisis intervention principles, recovery-based practice, and concurrent disorders.


Additionally, you should possess excellent communication and interpersonal skills, be able to collaborate effectively with stakeholders within and outside the Centre, and have demonstrated ability to work with clients from diverse backgrounds. Bilingualism (French/English) and/or proficiency in a second language would be an asset.
