Job Description

We are seeking an experienced Mental Health Rehabilitation Specialist to join our Assertive Community Treatment (ACT) program, IMPACT. This innovative program provides comprehensive support and rehabilitation services to adults with severe and persistent mental illness.

In this role, you will be part of a multidisciplinary team that includes psychiatrists, nurses, social workers, and other professionals working together to provide holistic care and support to our clients.

Your primary responsibility will be to work closely with clients, their families, and caregivers to develop personalized treatment plans that address their unique needs and goals.

  • Conduct assessments and develop treatment plans that focus on minimizing the impact of mental illness on clients' lives
  • Collaborate with interdisciplinary teams to provide comprehensive support and rehabilitation services
  • Work closely with clients, their families, and caregivers to ensure seamless transitions and ongoing support
