Social Navigator

We are seeking a highly skilled and compassionate individual to join our team as a Social Navigator - Health and Wellness Advocate at Kílala Lelum Health and Wellness Cooperative. This is an exciting opportunity to make a meaningful difference in the lives of our members and contribute to the delivery of high-quality, culturally-safe, and trauma-informed care.
Key Responsibilities- Care Coordination: Provide comprehensive care coordination services to members who are adversely affected by the social determinants of health and experience health challenges, including chronic disease, substance use, and mental illness.
- Interdisciplinary Collaboration: Work collaboratively with an interdisciplinary team, including Indigenous Elders, Physicians, Nurses, Allied Health professionals, and Outreach Workers, to deliver a plan of care and provide direct care activities.
- Community Engagement: Establish and maintain therapeutic relationships with members based on respect, cultural safety, trauma-informed practice, and harm reduction principles.
- Documentation and Reporting: Complete wellness checks, document relevant information, and work within care teams as necessary.
- Emergent or Time-Sensitive Care: Perform emergent or time-sensitive care as required, including appointment accompaniments.
- Caseload Management: Take on member caseload, implement/improve plans of care, and ensure appropriate maintenance.
- Education and Experience: Bachelor of Social Work and/or Master of Social Work preferred, plus two (2) years of recent related experience in Vancouver's DTES providing care through a holistic mental health, substance use, and primary care lens; or an equivalent combination of education, training, and experience.
- Cultural Competency: Ability to be culturally safe, culturally appropriate, and respectful with KL members.
- Communication and Collaboration: Ability to communicate effectively, both verbally and in writing, and work collaboratively with the team and community partners.
- Technical Skills: Experience with EMR, preferably OSCAR, would be beneficial.
- Knowledge and Commitment: Demonstrated commitment to culturally-safe, trauma-informed, member-centered, harm reduction, and recovery-oriented practices. Experience and knowledge of Indigenous peoples and Indigenous culture would be extremely beneficial.
The primary location will be in the Relational Care space, with the opportunity to work remotely where possible. This position will receive support from the Allied Health Lead in collaboration with the Relational Care Lead, and members of the Allied Health team.
