Job Title: Indigenous Patient Navigator

We are seeking a highly skilled and compassionate Indigenous Patient Navigator to join our team at PHSA. As a key member of our Indigenous Health Program, you will play a vital role in ensuring that Indigenous patients receive culturally safe and client-centered care.

Key Responsibilities:
  • Provide consultative and navigational services to patients referred to PHSA programs and services, supporting cultural interpretation and/or health literacy translation services.
  • Support patients in communications with physicians, nurses, and other frontline care staff, ensuring that their cultural needs are met.
  • Coordinate access and navigation of services for patients within the designated region, addressing cultural and systemic barriers.
  • Advocate for the needs of patients within the healthcare system, liaising with other community and regional health organizations to improve patient care.
  • Provide support and act as a resource for healthcare providers and other staff members, regarding the provision of culturally safe care.
Requirements:
  • Diploma or degree in a related health discipline, health or social sciences, or a patient care/clinical related discipline.
  • One to three years' recent related experience working in Indigenous healthcare.
  • Demonstrated knowledge and experience working with Indigenous communities and organizations.
  • Extensive knowledge of Indigenous peoples' demographics, languages, cultures, customs, and traditions.
  • Strong verbal and written communication skills.
  • Commitment to developing knowledge and understanding of legislative obligations and provincial commitments found in foundational documents, including Truth & Reconciliation Commission's Calls to Action (2015), In Plain Sight (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP), Reclaiming Power and Place Missing and Murdered Indigenous Women & Girls Calls for Justice (2019), the Declaration Act Action Plan, and Remembering Keegan: A First Nations Case Study.
