About the Role

We are seeking a highly skilled and compassionate individual to join our team as a Social Navigator at Kílala Lelum Health and Wellness Cooperative. As a key member of our interdisciplinary care team, you will play a vital role in promoting health and wellness equity in the Downtown Eastside (DTES) community.

Key Responsibilities
  • Care Coordination: Develop and implement comprehensive care plans for members who are adversely affected by the social determinants of health and experience health challenges, including chronic disease, substance use, and mental illness.
  • Team Collaboration: Attend regular team meetings and work closely with our interdisciplinary team, including Indigenous Elders, Physicians, Nurses, Allied Health professionals, and Outreach Workers, to ensure seamless care coordination and transitions.
  • Client-Centered Care: Provide direct care activities, such as psycho-social assessments, planning, implementing, evaluating, and documenting client care, while establishing and maintaining therapeutic relationships based on respect, cultural safety, trauma-informed practice, and harm reduction principles.
  • Knowledge and Skills: Possess a strong understanding of the social determinants of health, health management, and self-care management principles, as well as experience working with vulnerable populations in a community-based setting.
  • Communication and Collaboration: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including members, families, and community partners.
Requirements
  • Education and Experience: Bachelor of Social Work and/or Master of Social Work preferred, plus two (2) years of recent related experience in Vancouver's DTES providing care through a holistic mental health, substance use, and primary care lens.
  • Certifications and Registration: Current full registration with the British Columbia College of Social Workers, or willingness to obtain within 6 months.
  • Cultural Competence: Ability to work with Indigenous peoples and Indigenous culture, with a demonstrated commitment to culturally-safe, trauma-informed, member-centered, harm reduction, and recovery-oriented practices.
  • Language and Communication: Fluency in English, with the ability to communicate effectively, both verbally and in writing.
