Job Summary:

Key Responsibilities:

  • Provide one-on-one support and education to patients living with HIV, addressing their unique needs and concerns.
  • Work closely with healthcare providers to ensure patients receive timely and effective care.
  • Develop and maintain relationships with community-based organizations to provide patients with access to resources and services.
  • Assist patients in navigating mental health and substance use recovery systems.
  • Help patients overcome social determinants of health and other barriers to care.
  • Maintain accurate records and reports to track patient progress and outcomes.

Requirements:

  • 1 year of experience in a related field, such as healthcare, education, or social work.
  • Ability to work independently and as part of a team.
