Job Title: HIV Peer Navigator

Job Summary:

SEA MAR Community Health Centers is seeking a highly motivated and compassionate HIV Peer Navigator to join our team. As a Peer Navigator, you will play a vital role in helping patients living with HIV navigate the healthcare system and achieve viral suppression.

Key Responsibilities:

  • Provide one-on-one support and education to patients living with HIV, addressing their unique needs and concerns.
  • Work closely with healthcare providers to ensure patients receive timely and effective care.
  • Develop and maintain relationships with community-based organizations to provide patients with access to resources and services.
  • Assist patients in navigating mental health and substance use recovery systems.
  • Help patients overcome social determinants of health and other barriers to care.
  • Maintain accurate records and reports to track patient progress and outcomes.

Requirements:

  • 1 year of experience in a related field, such as healthcare, education, or social work.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Knowledge of HIV/AIDS and related health issues.

Preferred Qualifications:

  • Bilingual or multilingual skills.
  • Experience working with diverse populations, including those with HIV/AIDS.
  • Knowledge of community-based resources and services.
