We are seeking a highly skilled and compassionate individual to join our team as a Social Navigator at Kílala Lelum Health and Wellness Cooperative. As a key member of our interdisciplinary care team, you will play a vital role in delivering culturally safe and trauma-informed care to women in the Downtown Eastside (DTES) community.
Key Responsibilities- Provide Holistic Care: Develop and implement comprehensive care plans tailored to the unique needs of women, addressing health challenges such as chronic disease, substance use, and mental illness.
- Coordinate Care Transitions: Facilitate smooth transitions between services, ensuring seamless care coordination and communication with other health disciplines.
- Lead Social Navigation Programs: Oversee and facilitate Social Navigation drop-in programs, including Wednesday and Friday Night women's circles, and support the Mobile Outreach Program (MOP) at WISH Drop-in Centre.
- Build Therapeutic Relationships: Establish and maintain strong, respectful relationships with women based on cultural safety, trauma-informed practice, and harm reduction principles.
- Document and Report: Accurately document client care and progress, and escalate concerns to the care team as necessary.
- Education and Experience: Bachelor of Social Work required, with a Master of Social Work strongly preferred. Minimum two years of recent experience in Vancouver's DTES providing care to women through a holistic mental health, substance use, and primary care lens.
- Credentials and Registration: Current full registration with the British Columbia College of Social Workers, or a willingness to obtain within six months.
- Cultural Competency: Experience working with women, particularly those who have experienced trauma, violence, or marginalization, is strongly preferred. Completion of a cultural competency course or willingness to participate required.
- Work Environment: The Social Navigator will work at all Kilala Lelum sites, including the MOP van, with co-working office space at 687 Powell Street.
- Flexibility and Availability: 0.8 FTE (32 hours per week), with some flexibility required to meet the specific needs of women in the community.
- Competitive Salary: Hourly salary based on the Kilala Lelum wage grid, commensurate with local health authorities.
