Dietitian - Primary Care

We are seeking a highly skilled and experienced Dietitian to join our Primary Care team at Northern Health Authority. As a Dietitian, you will play a vital role in providing high-quality patient care and contributing to the development of a person and family-centered model of care.
Key Responsibilities- Develop and implement individualized nutrition plans for patients with various health conditions, including diabetes, kidney disease, and chronic diseases.
- Collaborate with interprofessional teams to provide comprehensive care and support to patients and their families.
- Conduct clinical assessments, treatment planning, and follow-up care to ensure optimal patient outcomes.
- Stay up-to-date with the latest research and evidence-based practices in nutrition and dietetics to provide the best possible care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Baccalaureate degree in nutrition and dietetics.
- Successful completion of an accredited dietetic internship program (or equivalent acceptable to the facility).
- Registrant of the College of Dietitians of British Columbia.
- Two years of clinical nutrition experience in a healthcare environment or an equivalent combination of education, training, or experience.
- Practice-specific requirements and experience in programs such as diabetes education, nutrition support, and kidney and chronic disease management programs.
- Valid BC Driver's License.
- Experience working in a primary care setting.
- Knowledge of electronic medical records and practice management systems.
- Ability to work independently and as part of a team.
