Dietitian - Primary Care

We are seeking a highly skilled and experienced Dietitian to join our Primary Care team at Northern Health Authority. As a Dietitian, you will play a vital role in providing high-quality patient care and contributing to the development of a comprehensive plan of care.
Key Responsibilities- Develop and implement individualized nutrition plans for patients with various health conditions, including diabetes, kidney disease, and chronic diseases.
- Conduct comprehensive assessments of patients' nutritional needs and provide evidence-based recommendations for dietary changes.
- Collaborate with interprofessional teams to develop and implement care plans that address patients' physical, emotional, and social needs.
- Provide education and support to patients and their families on healthy eating habits, meal planning, and nutrition-related topics.
- Stay up-to-date with the latest research and best practices in nutrition and dietetics, and apply this knowledge to improve patient outcomes.
- Baccalaureate degree in nutrition and dietetics.
- Successful completion of an accredited dietetic internship program (or equivalent acceptable to the facility).
- Registrant of the College of Dietitians of British Columbia.
- Two years of clinical nutrition experience in a health care environment or an equivalent combination of education, training or experience.
- Practice-specific requirements and experience in programs such as Diabetes Education, Nutrition Support, and Kidney and Chronic Disease Management Programs.
- Valid BC Driver's License.
- Experience working in a primary care setting.
- Knowledge of community resources and services related to nutrition and health.
- Ability to communicate effectively with patients, families, and healthcare teams.
