Nurse Practitioner

We are seeking a highly skilled and dedicated Registered Nurse to join our team in Primary Care at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities in Primary Care settings.
- Develop and implement care plans in collaboration with the interprofessional team, taking into account the patient's primary care plan and personal goals of care.
- Work effectively with patients, families, and caregivers to promote health, prevent illness, and manage chronic conditions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in Primary Care.
- Collaborate with other healthcare professionals to provide comprehensive care and support to patients and their families.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings, including experience developing and providing formal education to patients, families, the general public, or an equivalent combination of training and experience.
- Current valid BC Driver's License.
- Experience working in a Primary Care setting, preferably in a rural or remote community.
- Knowledge of population health and public health principles, including health promotion and disease prevention.
- Ability to communicate effectively with patients, families, and healthcare teams in a culturally sensitive and respectful manner.
