We are seeking a highly skilled Clinical Pharmacist to join our team in Primary Care. As a key member of our interprofessional team, you will provide clinical pharmacy services to patients, working directly with them to identify and resolve actual and potential drug therapy problems.
Key Responsibilities- Provide comprehensive medication management to patients in a team-based primary care setting
- Collaborate with healthcare professionals to develop and implement patient-centered care plans
- Conduct initial and ongoing patient assessments, using clinical and diagnostic reasoning to inform practice decisions
- Teach patients and others about medication management and health promotion
- Integrate best practice and current research evidence to support professional practice decisions and actions
- Current registration with the College of Pharmacists of British Columbia
- Minimum 2 years recent, related patient care experience, ideally in a team-based primary care setting or equivalent
- Valid Class 5 drivers' license and access to a personal vehicle for business-related purposes
