We are seeking a dedicated and compassionate Registered Nurse to join our team in Primary Care. As a Primary Care Nurse, you will be an integral part of our interprofessional team, providing high-quality patient care and working collaboratively with other healthcare professionals to deliver exceptional patient outcomes.
Key Responsibilities:- Provide clinical follow-up and care to patients in accordance with their primary care plan and personal goals of care.
- Develop and implement individualized care plans in collaboration with the interprofessional team.
- Conduct assessments and provide nursing care through appropriate interventions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
- Comprehensive benefit packages, including extended health/dental and a municipal pension plan.
- Four weeks vacation with one year of continuous service.
