Nurse Nutrition Specialist

In this dynamic position as a Nurse Nutrition Specialist, you will be an integral part of our food and nutrition department team at Southbridge Care Homes. Your primary focus will be on ensuring our residents receive nutritious and delicious meals prepared with care.
Responsibilities- As a member of the Clinical Team, you will consult with and advise the Food Service Manager and Director of Care concerning nutrition and hydration program needs. This includes nutritional assessment, care planning, implementation, and follow-up for residents.
- You will complete nutritional assessments for each resident using RAI MDS 2.0 as required, determining individualized diets and other nutritional interventions to meet their needs by involving the resident, family members, and care team.
- Developing, implementing, and evaluating nutritional care plans that identify nutritional concerns, goals, and interventions for all residents is a key responsibility.
- Maintaining excellent communication with the Food Service Manager regarding changing resident needs is essential.
- You will review seasonal menus for nutritional adequacy and provide written approval.
- Educating residents, family members, and team members on nutrition and hydration recommendations is a vital aspect of this role.
- You will participate in interdisciplinary teams, representing Southbridge through regional and professional activities as appropriate.
- Participating in committees related to resident care, facility operation, and quality indicators is also expected.
- A Registered Dietitian eligible for membership with the College of Dietitians of Ontario, with extensive knowledge of geriatric nutrition.
- 3-5 years of experience in long-term care settings are preferred.
- Proficiency in computer applications, Microsoft Office, and the internet, with experience in RAI MDS 2.0 desirable and familiarity with software PointClickCare an asset.
- Excellent organizational, time management, and multi-tasking skills are necessary.
- Knowledge of Occupational Health & Safety practices, principles, and legislation is required.
- Excellent oral and written communication skills are essential.
- A customer-focused attitude with demonstrated ability to build and maintain effective team relationships with staff and residents is crucial.
We offer a competitive salary range of $80,000 - $110,000 per year, based on experience, plus benefits including health, dental, and vision insurance, retirement savings plan, and opportunities for professional growth and development.
