Dietitian Nutrition Specialist

We are seeking a skilled Dietitian to join our dynamic Food and Nutrition Department team at Southbridge Care Homes.

This is a permanent part-time position with a competitive salary of $**80,000 - $100,000 per year**.

• As a member of our Clinical Team, you will consult with and advise the Food Service Manager and Director of Care on nutrition and hydration program needs.
• You will be responsible for nutritional assessment, care planning, implementation, and follow-up for residents.
• You will complete nutritional assessments for each resident using RAI MDS 2.0 as required.
• You will determine individualized diets and other nutritional interventions to meet resident needs by involving residents, family members, and care teams.
• You will develop, implement, and evaluate nutritional care plans that identify nutritional concerns, goals, and interventions for all residents.
• You will maintain excellent communication with the Food Service Manager regarding changing resident needs.
• You will review seasonal menus for nutritional adequacy and provide written approval.
• You will educate residents, family members, and team members on nutrition and hydration recommendations.
• You will act as a member of our Interdisciplinary Team, representing Southbridge through participation in regional and professional activities.
• You will participate in interdisciplinary committees related to resident care, facility operation, and quality indicators.

• You must be a Registered Dietitian eligible for membership with the College of Dietitians of Ontario.
• You should have extensive knowledge of geriatric nutrition.
• Three to five years' experience in long-term care settings is preferred.
• You should be proficient in computer applications, Microsoft Office, and the internet.
• Experience with RAI MDS 2.0 is desirable, and familiarity with PointClickCare software is an asset.
• You should possess excellent organizational, time management, and multi-tasking skills.
• You should have knowledge of Occupational Health & Safety practices, principles, and legislation.
• Excellent oral and written communication skills are essential.
• You should demonstrate a customer-focused attitude and ability to build effective team relationships with staff and residents.
