We are seeking a Registered Practical Nurse to coordinate and oversee the delivery of care to residents in a long-term care setting.

The ideal candidate is a warm and loving person who makes meaningful connections with those they are caring for, with a strong moral compass and a commitment to teamwork.

The Registered Practical Nurse reports to the Director of Care and is expected to provide direct supervision, leadership, and education to other members of the healthcare team.

The successful candidate will have a passion to make a difference and a commitment to delivering high-quality care to residents.

Key Responsibilities:

  • Develop and modify plans of care in response to residents' individualized health care needs and changes in health care status
  • Liaise with other healthcare personnel, agencies, and families
  • Maintain good relationships with Medical/Support/Home staff, residents, families, and the general public
  • Supervise all activities on the unit and take charge as required
  • Monitor performance and complete performance evaluations for PSW's and RPN's
  • Direct, assign, and evaluate the work of PSW's
  • May be in charge of the Home as required
  • Utilize leadership skills to identify problems and problem-solve on the unit
  • Monitor that care is delivered consistently with the resident Bill of Rights
  • Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported
  • Required to participate in the improvement of indicators and achieve satisfactory results
  • Order and ensure nursing supplies and equipment are properly and efficiently used
  • Actively listen to residents and family concerns and take appropriate actions to resolve the issue in accordance with policies of the Home
  • Conduct unit rounds at the beginning and end of shift
  • Process Physician orders and administer medications and treatments in compliance with the policies of the Pharmacy provider, the Home, and the Standard of the College of Nurses
  • Document and report all errors and omissions
  • Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and the policies of the Home
  • Assess changes in health status and report changes to Substitute Decision Maker (SDM) or POA, and interdisciplinary team
  • Complete health care assessments (RAI-MDS and other assessments), develop and update plans of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team
  • Demonstrate understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home

Qualifications:

  • Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Basic CPR
  • Experience in long-term care setting an asset
  • Ability to work with and relate to interdisciplinary team members
  • Well-developed written and oral communication skills
  • Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired
  • Ability to show evidence of being focused on people's feelings when interacting and knowing how to turn a task into positive social interaction
  • Ability to undertake and meet all legal requirements within a context of promoting individuals' rights and measuring/reducing elements of risk
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
  • Two supervisory references required
  • COVID-19 vaccination is a condition of employment
