Job Summary:

Medavie Health Services is seeking highly skilled Social Workers and Registered Nurses to join our team as Mobile Mental Health Team Members. As a key member of our interdisciplinary team, you will provide mental health and addiction support to individuals and families in rural communities across Southwest Ontario.

Key Responsibilities:
  • Conduct in-home client visits with Mobile Mental Health Team members
  • Provide client support through Tele-Support access line
  • Assess client's condition and presenting problem, and contributing factors
  • Conduct mental health and risk assessments to facilitate client rapport
  • Collaborate and share information with law enforcement and/or Paramedic Services
  • Recommend and facilitate referrals to appropriate alternative resources
  • Employ a technical knowledge of psychotropic medication
  • Gather information from interviews with the client and collateral information from family and other professionals
  • Use substantive knowledge of numerous areas to formulate clinical impressions and recommendations
  • Ensure that evidence-based practices are embedded into client care
Requirements:
  • Considerable clinical experience in mental health and addiction settings
  • Registered and in good standing with the Ontario College of Nurses
  • Ability to provide initial psychological interviews and assessments
  • Experience on interdisciplinary teams, in program development and case management
  • Independent critical thinking, clinical judgment, and independent decision-making skills
  • Ability to work effectively as a reliable member of a team
  • Valid Driver's license in good standing
  • Good previous work attendance record
