Job Summary:

Ambulatory Services at Baycrest Health Sciences has an opportunity for a Physiotherapist to join the team. This position will support Ambulatory Services Day Hospitals and Outreach Teams, working creatively in various community environments and programs, including outpatient, day hospitals, outreach teams, community housing, primary care clinics, and online platforms for virtual access to care.

Key Responsibilities:
  • Conduct clinical assessments, care planning, treatment interventions, and discharge planning for complex geriatric, neurological, and musculoskeletal populations.
  • Participate in teaching and mentoring physiotherapy and physiotherapy assistant students.
  • Assign patient plans of care and provide direction, supervision, and monitoring of physiotherapy assistants.
  • Collaborate with the inter-professional team to implement client-focused care plans and coordinate discharge planning.
  • Provide support and consultation to other disciplines on the team.
  • Assign tasks to OTA/PTA when appropriate.
  • Support a systematic, evidence-based approach to client care, program development, and evaluation, in collaboration with inter-professional team members, volunteers, and the Physiotherapy clinical team.
Qualifications:
  • Licensure in good standing with the College of Physiotherapists of Ontario.
  • Membership or eligibility for membership with the Canadian Physiotherapy Association (an asset).
  • Experience working with geriatric populations with multiple medical conditions and/or cognitive impairment (preferred).
  • Confirmed status as ADP authorizer required.
  • Expertise in clinical assessments of elderly clients.
  • Ability to lead groups and plan/executes knowledge translation/adult learning sessions with clients, caregivers, family members, and inter-professional team members.
  • Experience in providing clinical instruction and teaching to physiotherapy students (an asset).
  • Previous experience and/or interest in developing evidence-based programs (an asset).
  • Excellent interpersonal, communication, time management, and problem-solving skills.
  • Access to transportation (e.g., current driver's license and car) required.
  • Proficient computer skills – Meditech, MDS, Microsoft Word, PowerPoint, and Excel required.
