About the Role

Closing the Gap Healthcare is seeking a highly skilled and compassionate Wound Care Specialist to join our team in various locations across Mississauga, Halton, GTA, Central, and Dufferin. As an integral part of our community-based healthcare services, you will be responsible for providing high-quality nursing care to clients with wound, ostomy, and continence needs.

Job Summary

We are looking for a dedicated and motivated individual who is passionate about delivering patient-centered care. Your expertise in wound management, ostomy care, and continence related nursing diagnoses will enable you to assess, plan, collaborate with clients, family members, and other healthcare professionals to establish and/or update care plans. Your goal will be to implement and evaluate client care with every encounter and provide clinical advocacy.

Responsibilities
  • Assess clients for appropriateness of virtual consults
  • Provide high-quality, appropriate nursing care to individuals with actual and potential skin integrity issues related to wound management, ostomy care, and continence related nursing diagnoses
  • Collaborate with clients, family members, and other healthcare professionals to establish and/or update care plans
  • Implement and evaluate client care with every encounter
  • Provide clinical advocacy
Requirements
  • Hold a certificate for successful completion of a wound, ostomy, and continence program (NSWOC) from a recognized educational institution; or Hold a certificate for successful completion of a Wound Ostomy Continence Nurse (
