About the Role:As a Community Care Specialist, your key responsibilities will include:
- Providing scheduled visits in the community to assess and meet the needs of patients within your scope of practice.
- Delivering high-quality patient care, using appropriate assessment tools and techniques to ensure safe and effective treatment.
- Collaborating with patients, families, and healthcare teams to develop and implement personalized care plans.
- Maintaining accurate and up-to-date patient records, including progress notes and treatment plans.
To be successful in this role, you will require:
- A current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- A valid driver's license and access to a reliable vehicle.
- Experience in delivering community care, preferably with a focus on vulnerable populations.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with patients and families.
- A strong commitment to providing exceptional patient care and service excellence.

- An estimated annual salary of $70,000 - $90,000, depending on experience.
