About the Role
We are looking for a compassionate and detail-oriented individual who can work effectively in a dynamic healthcare environment. Your primary responsibilities will include assessing patient needs, developing and implementing personalized care plans, and collaborating with interdisciplinary teams to ensure seamless care delivery.
Key Responsibilities
- Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing tailored care plans to meet those needs.
- Care Coordination: Collaborate with healthcare professionals, families, and caregivers to ensure comprehensive and coordinated care delivery.
- Communication and Documentation: Maintain accurate and up-to-date records of patient interactions, progress, and concerns, while communicating effectively with patients, families, and colleagues.
