Patient Service Coordinator

The Patient Service Coordinator–Dedicated Program, under the direction of the Patient Services Supervisor, Specialty Nursing Services, plays a critical role in supporting patients with their care coordination needs. This position is responsible for scheduling patient services within the Specialty Nursing Network, ensuring timely and professional communication with patients, and meeting key performance indicators.
Main Responsibilities:
- Manage and document patient inquiries regarding requests for service, providing exceptional customer service and empathy.
- Liaise with program managers and specialists, scheduling coordinators, and patients to efficiently schedule patient service, ensuring seamless coordination and minimal disruption to patient care.
- Assess patient service needs in consultation with Supervisors, utilizing clinical knowledge and judgment to determine the most suitable solutions.
- Complete data entry and maintain accurate current scheduling documentation, ensuring compliance with Bayshore HealthCare policies and procedures.
- Schedule licensed employees for care and service assignments, taking into account patient needs, employee qualifications, and skill sets to optimize scheduling efficiency.
- Notify patients of ongoing schedules, maintaining open communication and transparency throughout the process.
- Meet required scheduling key performance indicators, driving continuous improvement and quality enhancement.
- Handle and document patient concerns and complaints in a timely manner, escalating as necessary to ensure prompt resolution.
- Participate in ongoing internal and/or external continuing education activities, staying up-to-date on industry best practices and Bayshore HealthCare's Quality Management System.
- Maintain confidentiality of patient and corporate information, adhering to Bayshore HealthCare's strict privacy protocols.
Qualifications
- Secondary School Diploma required; Bilingual English/French (oral and written) essential; completion of a recognized program such as Licensed Registered Practice Nurse or Medical Terminology course preferred.
- A minimum of two years related customer service experience, preferably in healthcare; previous scheduling experience advantageous.

- Ability to manage relationships with exceptional customer service, demonstrating empathy and understanding.
- Exceptional organizational and interpersonal skills, with the ability to handle difficult situations with tact and professionalism.
- Proven ability to work both independently and as part of a team, contributing to a collaborative and supportive work environment.
- Ability to meet deadlines and demonstrate a strong commitment to continual learning and professional growth.
Estimated Salary Range: $45,000 - $60,000 per year, commensurate with experience and qualifications.
