Palliative Care Nurse

We are seeking a skilled Registered Practical Nurse (RPN) to join our team as a Palliative Care Nurse. In this role, you will provide compassionate care to patients in their homes, working collaboratively with RNs to deliver high-quality care.
Key Responsibilities- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques
- Develop and implement care plans, monitoring patient progress and adjusting as needed
- Build rapport and trust with patients, engaging them in their care and involving them in decision-making
- Maintain accurate and up-to-date patient records
- Registration with the CNO as a Registered Practical Nurse in good standing
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is strongly preferred
- Valid driver's license and access to a vehicle
- Reliability and flexibility, with willingness to travel to patient residences
