About the Role
We are seeking a skilled Palliative Registered Nurse to join our team at ParaMed Home Health Care. As a Palliative RN, you will provide compassionate care to patients in their homes, working with patients of all ages and complexity of care needs.
Key Responsibilities
- Assess patients' individualized needs using appropriate assessment tools and techniques
- Execute and continuously monitor care plans to ensure optimal patient outcomes
- Engage patients using a collaborative approach, building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
Requirements
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver's license and access to a vehicle
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is strongly preferred
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
